Compensation/Pension

Please contact the County Veteran Service Officer first to discuss eligibility, benefits and required supporting documents needed to support claim.

Download and complete required forms:

  • VA FORM 21-526EZ (Veteran's Application for Compensation and/or Pension)
  • VA FORM 21-22 (Appointment of Veteran Service Organization as Claimants Representative)
  • DD214 (copy 4) Required to submit claim
  • VA FORM 21-2680 (Examination for Housebound Status or Permanent Need for Aid & Attendance)

To apply, bring DD 214 (copy 4), VA 21-526EZ, VA 21-22 to County Office or call to make an appointment. A County Veteran Service Officer will assist in completing the application process and advise you of future actions required.

You may be required to provide additional documents such as a marriage certificate, death certificate and financial information.

Widow Pension / Aid & Attendance
