Required Documentation to Apply
Escambia County Neighborhood & Human Services Department reserves the right to request additional documentation deemed necessary to determine eligibility.
- Proof of qualifying COVID-19 impact / financial hardship
- Proof of identification for applicant/co-applicant and all household members over 18
- Proof of social security card(s) – ALL household members
- 2020 Federal Income Tax Returns for each household member over 18
- Current proof of income – ALL sources of household members income must be disclosed
* Last 6 consecutive check stubs
* Current award letter of monthly SS, SSI, SSD, SSA, etc.
* Proof of other non-employment income: VA, retirement, military, pension, child support, alimony, unemployment
* Completed year to date profit and loss statement for any adult household member self employed
- If applicable, Zero Income Form
- Bank statements (last 6 consecutive months) ALL bank accounts including minor accounts
- Copy of final divorce decree; court order for child support with current printout for each child; alimony
- Proof of loss of income, reduction of hours or unemployment as a result of COVID 19 pandemic on or before March 1, 2020
- Most recent Florida Power and Light and/or other utility providers in the area bill (account must be held in applicant/co-applicant name)
- Completed W9 from lender
- Copy of Homeowners or Condominium Association statement reflecting delinquency
- Mortgage statements from March 2020 or when delinquency began but NOT prior to March, 2020
- Most recent mortgage statement showing delinquent amount owed (including property tax and homeowner’s insurance if included in mortgage payment)
- Forbearance letter or payment agreement from bank, if applicable
